“Sleep it off” isn’t just an old wives’ tale. It’s biology.
Sleep and the immune system are so closely linked that it’s hard to understand one without the other. The immune system is at its most active while you sleep, and sleep deprivation weakens it in ways that are well documented and clinically meaningful.
What the immune system does during sleep
During sleep, particularly deep sleep, a range of immunological processes take place that can’t happen as effectively while you’re awake.
Cytokine production. Cytokines are signalling molecules that coordinate the immune response, communicating between immune cells and directing attacks on pathogens. Many cytokines are produced primarily during sleep. Interleukin-1 and TNF-alpha, both sleep-promoting and immune-stimulating, are among them.
T-cell activation. T-cells are a key part of the adaptive immune system — they recognise and attack infected cells. Research shows T-cells’ ability to bind to their targets is significantly higher during sleep than while awake. The mechanism involves a drop in stress hormones (adrenaline and noradrenaline) that would otherwise suppress T-cell activation.
Immune memory formation. Just as the brain consolidates cognitive memories during sleep, the immune system consolidates immunological memory — the system that keeps you protected against illnesses you’ve already had, and that makes vaccines work.
Sleep deprivation weakens immunity — the numbers
The link is well quantified in research.
A study from Carnegie Mellon University exposed healthy volunteers to a cold virus after tracking their sleep patterns. The result: those who slept less than six hours had a substantially higher risk of catching a cold than those who slept seven hours or more.
Another study found that after a single night of total sleep loss, natural killer cell activity — the immune system’s first line of response — dropped sharply.
Vaccine studies consistently show that sleep deprivation in the days after vaccination reduces antibody response markedly. People who slept well after a flu vaccine had notably higher antibody levels weeks later.
Immunity and sleep quality — a two-way link
The relationship isn’t one-directional. The immune system actively sends sleep-promoting signals of its own.
When you’re ill, the body increases your sleep need because sleep accelerates recovery processes. Fever brings sleepiness because the hypothalamus — which regulates both body temperature and sleep — activates the sleep process as part of the illness response. Cytokines produced during infection are directly sleep-promoting.

Chronic sleep loss and inflammation
Chronic sleep deprivation — regularly sleeping under seven hours over time — is associated with raised systemic inflammation, measured through biomarkers like C-reactive protein (CRP) and interleukin-6. Chronic inflammation is an underlying factor in cardiovascular disease, type 2 diabetes, cancer and neurodegenerative disease.
That’s one of the mechanisms behind the epidemiological finding that chronic sleep loss raises the risk of these conditions — not just correlation, but a plausible causal pathway through inflammation.
Sleep and dreams during illness
Many people notice their dreams become especially intense and bizarre during illness, fever in particular. That’s not a coincidence. Fever raises brain activity and can produce more vivid, hyperreal and sometimes hallucinatory dream scenarios. Some cultural beliefs about visionary dreams during illness likely have their roots in this phenomenon.
Practical takeaways
You can’t compensate for sleep loss with supplements, vitamins or diet — the immune system needs the sleep process itself, not just the raw materials it draws on.
Prioritise sleep during illness and recovery. It isn’t laziness; it’s immunologically necessary.
Prioritise sleep after vaccination. Sleeping well for the couple of nights after a flu or covid vaccine supports a better antibody response.
